USCIS may require that you appear for an interview or provide biometrics (fingerprints, photograph, and/or signature) at any time to verify your identity, obtain additional information, and conduct background and security checks, including a check of criminal history records maintained by the FBI, before making a decision on your request.
Provide the name of the country where you are a citizen and/or national.
List all your absences from the U.S. since June 15, 2007. Include information about all your departure and return dates, and the reason for your departures.
If arrested/charged with any felony or misdemeanor in the U.S., or any crime abroad, submit certified arrest and disposition records or an official statement if no charges were filed. If vacated, sealed, or expunged, submit a court order or statement showing no record. Minor traffic violations need not be submitted unless alcohol/drug-related.
With your DACA renewal, submit only new documents on removal proceedings or criminal history not previously provided. Submit if you are in exclusion, deportation, or removal proceedings (unless administratively closed), or if you were charged/convicted of a felony or misdemeanor not already submitted. Application for Employment Authorization.
Select the item number that best describes the reason for your application.
Full legal name, previously used names, date of birth, place of birth, sex, marital status, and your U.S. mailing and physical addresses.
Your alien registration number, if available.
This number can be found on the profile page of your USCIS account.
Provide copies of your previous EADs, if available.
If the SSA ever issued a Social Security card to you in your name or a previously used name, then you must provide the SSN from your card. If you request a new or replacement Social Security card, you must provide your father’s and mother’s family and given names at birth.
Provide the name of the country or countries where you are currently a citizen or national.
If CBP or USCIS issued you a Form I-94 (i.e., an Arrival-Departure Record), then you must provide your Form I-94 number (otherwise known as a Departure Number).
If you used a passport or travel document to travel to the U.S., you must enter either the passport or travel document information.
Provide the location and date on which you last entered the U.S.
Provide your status when you last entered the U.S. as well as your current immigration status.
Provide your SEVIS number, if applicable.
For DACA and DACA renewals, USCIS requires you to file Form I-765 under category (c)(33).
